I agree and indemnify and hold Lake Charles Ward 3 Recreation and its employees, affiliates and volunteers harmless from liability, loss, cost or expense that may occur while participating in activates and/or programs given by Lake Charles Ward 3 Recreation. I understand that failure to display appropriate behavior may result in dismissal from Ward 3 Recreation programs and forfeit of any registration fees submitted for participation in program. Inappropriate behavior and conduct includes, but not limited to profanity, abusive language, inappropriate attire, smoking and use of alcohol or drugs, aggressive behavior or taunting of referees and/or program staff, or criminal conduct of any type. Failure to sign team participation form will automatically cause a team forfeit if a team challenges a player’s participation.
